Cats Switched to a Renal Diet Early Progressed 41-46% Less: What a 1,430-Cat Study Says About Early Detection
A JAVMA study of 1,430 cats (March 2026) found that starting a therapeutic renal diet at IRIS stage 1-2 cut the risk of progression by 41-46% and all-cause mortality by 30% over three years. The Numbers From 1,430 Cats
A study published March 11, 2026 in the Journal of the American Veterinary Medical Association (JAVMA 264:5) followed 1,430 cats with early-stage chronic kidney disease (CKD), comparing 839 cats prescribed a veterinary therapeutic renal diet against 591 that were not.
The findings:
- Cats for whom a renal diet was a consistent part of the treatment plan were 41-46% less likely to progress to a more advanced CKD stage (holding across Stage 1, Stage 2-WRI and Stage 2-ARI)
- Their risk of all-cause mortality was 30% lower in the three years after diagnosis
- Among cats that died during the study, the diet group survived 20% longer

One caveat worth stating plainly: this is a retrospective analysis of clinical records, not a randomized controlled trial, so selection effects cannot be fully excluded. Even so, the sample size and the size of the effect make it a serious argument for intervening early.

The Real Finding Is Not the Diet — It Is the Timing
Therapeutic renal diets are not new. What is new here is that the study puts a number on the difference between starting one early versus starting one late.
Renal diets have often been treated as something you begin once kidney disease has clearly advanced. That instinct is understandable: a cat in the early stages looks completely well, and it is hard for an owner to justify changing the food of a cat that seems fine.
This study argues directly against that instinct. The cats that started while they still looked healthy progressed more slowly and lived longer.
Put differently, what mattered was not only what was in the bowl. It was that the disease had been caught while options still existed. Cats found at an advanced stage were never eligible for this study in the first place.
Japan's Pending AIM Drug Points in the Same Direction
In Japan, the AIM protein therapy for feline kidney disease was filed for approval with the Ministry of Agriculture on April 24, 2026 under the name FeliAIM, and is currently under review (details: AIM drug update).
What is striking is that the condition experts keep attaching to that drug is the same one: catch it early. Trials were run in stage 3 cats, but the greatest expected benefit is at stages 1-2, while effectiveness at stage 4 is expected to be limited — because AIM halts progression rather than regenerating damaged tissue.
So an old tool (diet) and a new one (a novel drug) point to exactly the same conclusion:
Only the cats found early can take full advantage of either.
There is nothing wrong with waiting for the drug. But whether you can say "my cat is still at stage 1" on the day it is approved depends less on the drug’s timeline than on the records you start keeping today.
Early CKD Shows You Almost Nothing
The hard part about early CKD is that the cat looks fine. Kidney function is typically two-thirds gone before obvious symptoms appear.
But it does show up in numbers and records.
Signs you can catch at home:
- Drinking and urinating more (the most common early change — larger litter clumps, the water bowl emptying faster)
- Gradual weight loss (only visible month over month; by the time it feels obvious in your arms, it is late)
- Inconsistent appetite, less grooming, more sleeping
